Introduction & Overview

In 1905 Dr. Best first described a condition affecting the macular region of the retina (located in the eye). The retina and its component photoreceptor cells are essential to vision as they convert light into electrical impulses and then transfer these impulses to the brain via the optic nerve. The macula is the central part of the retina responsible for fine visual detail and color perception.

The term macular degeneration refers to a group of disorders that affect the central portion of the retina and, as a result, the center of the field of vision. The most common forms of this disease usually affect patients over the age of 65 and are collectively the most frequent cause of legal blindness in developed countries.

BMD is an autosomal dominant disorder. With such genetic conditions it is likely that there would be several affected individuals in successive generations but BMD can be erratic and may miss a generation.

Although the age of onset of BMD can vary, it is usually diagnosed during childhood or adolescence. In the initial stages, a bright yellow cyst (fluid-filled sac) forms under the retinal pigment epithelium (RPE) beneath the macula. Upon examination, the cyst looks like a sunny-side-up egg. Despite the presence of the cyst, visual acuity may remain normal or near normal (between 20/30 and 20/50) for many years. Peripheral (side) vision usually remains unaffected.

In many individuals with BMD, the cyst eventually ruptures. Fluid and yellow deposits from the ruptured cyst spread throughout the macula. At this point the macula has a scrambled egg appearance. Once the cyst ruptures, the macula and the underlying RPE begin to atrophy causing further vision loss. As a result, central vision tends to deteriorate to about 20/100 late in life. However, BMD disease does not always affect both eyes equally. Many individuals retain useful central vision in one eye with a visual acuity of about 20/40 in the lesser affected eye.

In some cases, BMD does not progress far enough to cause significant central vision loss. However, retinal specialists can still detect the disease using sophisticated diagnostic tests that measure the function of the RPE and the retina. Individuals with BMD are also often farsighted. Although the farsightedness can be corrected with glasses, the other vision-affecting symptoms currently remain untreatable.

Phenotypic Expression of BMD

BMD is an autosomal dominant disease with a juvenile age of onset. BMD is diagnosed by its unique electrophysiologic symptoms, a depressed EOG (light peak to dark trough ratio *1.5) in the absence of an altered electroretinogram

The inside of the eye is lined by three layers of tissue, each critical for normal vision. The innermost layer (the one first struck by the light that enters the eye) is known as the retina and consists of a complex network of nervous tissue. Some of the cells in this layer (the photoreceptors) convert light into an electrical signal that is then amplified and processed by other cells before being sent to the brain via the optic nerve.

The central part of the retina–the macula–has a number of special structural features that allow images focused on it to be seen with very high resolution. (See Appendix 1) The middle layer is a one-cell-thick sheet known as the retinal pigment epithelium, or RPE. The RPE provides metabolic support for the photoreceptor cells and also removes old bits of cellular debris from the tips of the photoreceptor cells as they renew themselves. The layer farthest from the incoming light is a rich network of blood vessels known as the choroid. These vessels supply oxygen and nutrients to the retinal pigment epithelium and photoreceptor cells and carry away waste products.

In macular degeneration, clumps of yellowish cellular debris–possibly of retinal origin–gradually accumulate within and beneath the retinal pigment epithelium. These deposits are visible to the clinician looking inside the eye as small yellow dots known as drusen (singular: druse). (See Appendix 2) With the passage of time, patches of retinal pigment epithelial cells may die, resulting in bare spots known as geographic atrophy.

When the support functions of the RPE are lost, the photoreceptor cells overlying the areas of geographic atrophy cannot function and the vision from this patch of retina is lost. If these patches become large and involve the very center of the macula (the fovea), the individual’s visual acuity can fall to the point that he or she is considered legally blind. This atrophic phase of macular degeneration is sometimes referred to as “dry” macular degeneration.

In approximately 10 percent of patients with macular degeneration, the injury to the retinal pigment epithelium stimulates the choroidal blood vessels to grow up into the RPE and retina–seemingly in an attempt to heal the defects in these layers. This reparative response is very similar to those that occur elsewhere in the body in response to injury–such as scar formation after a cut on the skin. Unfortunately, the retina is such a complex and highly ordered tissue that the in-growth of these new blood vessels causes more visual loss than the original degenerative process. In fact, although only 10 percent of patients develop new blood vessels, this complication is responsible for most of the legal blindness associated with macular degeneration. The vascular phase of macular degeneration is sometimes called “wet” macular degeneration.

Because the new blood vessels (also known as choroidal neovascular membranes) can be so damaging, a wide variety of treatments have been tried to arrest their growth. By far the most successful to date has been the use of laser light to cauterize the blood vessels. Unfortunately, laser treatment has a number of significant drawbacks including a high recurrence rate, laser injury to the retina, and an inability to treat the majority of patients affected with neovascular membranes (because the lesions are too large or ill-defined when discovered).

Causes of Macular Degeneration

Physicians have wondered about the causes of macular degeneration for over a century. In the late 1800s, when doctors first began looking into eyes with ophthalmoscopes, they believed that the drusen they observed represented some type of infection, or at least inflammation, of the choroid. Even today, there is some evidence to suggest that the body’s immune system plays a role in the development of some forms of macular degeneration, especially the development of neovascularization.

An important group of possible causes of macular degeneration is mildly abnormal genes. It has been recognized for over a century that some forms of macular degeneration run in families. During the past 25 years, increasing evidence has been gathered to suggest that a significant fraction of macular degeneration has a hereditary basis.

This has important implications for understanding macular degeneration at the molecular level, as well as for designing improved treatments for the disease. When a disease like macular degeneration is caused by a dominant gene, a number of family members may be similarly affected. Such families can be studied by modern molecular genetic methods in ways that allow the causative gene to be identified.

In the past 10 years, the chromosomal locations of at least 10 genes that cause macular-degenerationlike conditions have been identified, and three of the genes have actually been identified. Unfortunately, none of these three genes causes a measurable fraction of typical late-onset macular degeneration, but the disease mechanisms are similar enough to the latter condition that scientists can already begin developing animal models of macular degeneration based on these genes to use in new treatment research.

The genetic approach is particularly appealing because if a genetic predisposition to macular degeneration can be identified, it raises the possibility that individuals can be tested for the predisposition early in life, and given some sort of treatment that will delay or prevent the onset of the macular disease. Such treatment has the potential to be safer, simpler, cheaper (and hence more widely available) than some of the other experimental treatments currently under development.

Areas of Promising Research

Since the majority of severe visual loss is caused by abnormal blood vessels growing beneath the retina, a massive effort is underway to identify methods of arresting this process. Any breakthrough in this area could be of immediate benefit to thousands of patients with macular degeneration. Strategies under investigation at this time include the use of drugs, growth factors, anti-growth factors, surgery, and radiation. All these strategies are designed to retard new blood vessel growth without significantly damaging the overlying retina (as occurs with conventional laser treatment).

In 2001, the mainstays of therapy for patients with macular degeneration include daily monitoring of the integrity of their central vision (usually by viewing a simple printed grid) as well as periodic visits to their eye doctor. Both of these strategies are designed to identify treatable new blood vessel membranes as early as possible. When such a growth is suspected, it is confirmed by an angiographic procedure. If the membrane has characteristics that have been shown to be favorable for treatment, laser photocoagulation is applied by an ophthalmologist who has had special training in the technique.

Patients who lose vision despite treatment, or who lose vision from atrophic disease, can often be significantly helped by a thorough low-vision-rehabilitation program. Such a program is administered by an optometrist or ophthalmologist with special training in low vision and consists of a number of integrated parts ranging from counseling to the prescription of special glasses, magnifiers and even electronic devices.

Mapping and Sequencing of BMD Gene

Many important human genes, including those causing diseases, cannot be easily studied with standard techniques of molecular biology, like cloning and sequencing. For most genetic diseases information concerning the gene function are not available and the biochemical basis of the pathogenetic mechanism of the disease is often limited. These obstacles hinder the progress of genetic studies with standard procedures. These difficulties can be overcome through the genetic linkage analysis. This paper has been plaguerized. The linkage analysis allows the identification of chromosomal localization of the disease-associated gene without having any hypothesis on its pathogenetic mechanism. The BMD disease gene (VMD2) has been localized to chromosome 11q12-q13.1 within an approximately 1.4 Mbp interval between markers at D11S1765 and uteroglobin (UGB). This region has been cloned in overlapping yeast artificial chromosomes (YACs) and P1 artificial chromosomes (PACs) as a prerequisite for the construction of a gene map of the BMD locus. A comprehensive characterization of these genes is required to identify possible candidates for the disease gene and consequently to isolate the defective gene by analyzing BMD patients for mutational changes.

To identify the gene defect, genetic linkage mapping was performed and has positioned the BMD locus to an approximately 1-2 cM interval in 11q12-13.1. The closest flanking markers are D11S1765 proximally and UGB distally. The entire interval has been cloned into overlapping YAC and PAC clones together covering approximately 1.4 Mbp. The contig (sequence of overlapping clones) provides the basis for the generation of a comprehensive transcript map from the BMD locus. In pursuit of this goal, researchers are currently using two strategies including the fine mapping of known genes and ESTs and direct tissue-specific cDNA selection. This report has been plaguerized. Thus far, another 14 novel transcripts have been added to the 6 genes (CD5, PGA, DDB1, FEN1, FTh2 and VMD2) localized within the minimal BMD region. In addition, researchers have determined their expression pattern in various human tissues including the retina and RPE. Several transcripts were identified that reveal an expression profile conceivable with an involvement of these genes in the pathogenesis of BMD. Their further characterization and mutational analysis in BMD patients is currently under way.

VMD2 encodes a previously unknown protein designated bestrophin. Although mutations in VMD2 in individuals with age-related macular dystrophy (AMD) are rare, mutations have been reported in up to 1.5% of individuals with AMD. Bestrophin is predicted to be a 585-aa protein with an approximate mass of 68 kDa.

Based on Northern blotting and in situ hybridization data, bestrophin is predominantly expressed in the RPE. Bestrophin shares homology with the Caenorhabditis elegans RFP gene family, named for the presence of a conserved arginine (R), phenylalanine (F), and proline (P) amino acid sequence motif. However, the function of the RFP genes is unknown. Computer-assisted structural analysis predicts that bestrophin is a transmembrane protein with four membrane-spanning ALPHA (insert greek symbol) helical domains, although a less likely five-transmembrane domain model recently has been proposed. There are no obvious targeting signals present in the amino acid sequence that would provide clues to its subcellular localization, and based on the four-transmembrane domain model, bestrophin has no obvious sites for posttranslational modifications such as N-glycosylation.

Implications for Treatment and Prevention

Knowledge of a disease-gene allows in principle for gene therapy. One of the advantages of the gene therapy is that it does not require a detailed understanding of the function of the gene or of its product. Were we to find specific mutations in macular dystrophies which are also associated with a subgroup of age-related macular degeneration, the pathogenetic mechanism could be investigated by using cellular cultures or transgenic animals. The progress already achieved and that researchers hope to attain in the field of molecular genetics will contribute increasingly to improve the management of patients affected by hereditary degenerative diseases. The risk of disease onset could thus diminish through prevention or adequate treatment.

I look forward to going to a place where great home-cooked meals are served. Sometimes eating in the places like Friday?s, Ale House, Rockaway?s, Apple Bee?s, and the Dugout bores me. Most of these places come with a razzle and dazzle appearance and annoying servants who are out to make a buck by kissing your feet with every desire. The reason why I like Iris?s restaurant so much is because of the usual family atmosphere where only the regulars go like me. Iris?s is also my favorite because of the scrumptious food at a cheap price that keeps coming due to great service unlike at the expensive franchises where food is sometimes forgotten about. The only set back to this homely environment is the amount of fluctuation between regular customers and the ?Snow Bird? customers. Oh, and if you are wondering a ?Snow Bird? is a term used to name migrators from the north especially Canada to Florida during the winter season.

In saying Iris?s restaurant is kept plain and simple I mean the place looks like a shack on the out side with a touch of antique d?cor of wood finished tables, chairs, booths, and more ?nick-knacks? on shelves inside. The restaurant does not look like it came out of Star Wars movie that is for sure. The Star Wars affect is something that would attract ?Snow Birds? anyway. That affects the price which goes up and that is not what a native to the area would look for. A local would look for a typical Sherry, one of the original waitresses, who always keeps the attitude moving by giving the old folks a few jokes and serving it up in style just for kicks. She does not need the loud music or shining lights to do that either. Other reasons are hard to come by but clothing is another key factor to a great atmosphere.

The owners and the staff all are dressed casually in a forest green golf shirt and khakis. They are not dressed to kill so their breasts fall out on your food while they serve you, like at Hooters. They say it gives them room to work their tails off all the time to keep the place spic and span like Mr. Cleans forehead. That includes the area where the customer can?t see also. An example is areas where food is about to be prepared.

Iris?s is different from any franchises located in the area because the food is freshly prepared and not prefabricated in some factory, sent out to the restaurants to be re-cooked (in a microwave), and then placed on a plate to look tasty. The cooks at Iris?s work double time on the orders of food instead gawking of the girls that walks in the front door. Cooks at a homely place hardly get an opportunity to drool because they are to busy getting the food out to the customers. In return for working so hard they hardly ever get complaints about getting the food out faster. The quaint smell of the food is so delightful to ones nose that it adds to the antique feeling, one you would get when you walk into an old-fashioned store front.

This is the kind of home cooked food I was looking for. The different entries come in a large variety of soups and salads to stake and potatoes. They are all made with the best of care and a taste that backs up the homely atmosphere. I like the Caribbean Jerk chicken the best, it?s something the cook added to the menu after I ordered it so much. My parents have tried everything on the menu from fish and chips, buffalo chicken salad, sweet potato fries, liver and onions, to daily specials like eggplant Parmesan, and grilled scallops, and all you can eat fish of the day. The service stays pretty much the same until the ?Snow Birds? show up and then all heck breaks out and normal specials and times to go and not be irritated change for the season.

This exception to ?Snow Birds? arriving makes everything seem to go bye differently. Even the owner tries to keep the common trends going with his regulars by placing, ?Snow Bird feed sold here,? on the large billboard out front.

It is a quiet place except when it isn?t jammed packed with people on Sundays, noon on weekdays, and early Saturdays during busy times and especially the winter season. I have been there on all of these occasions and there is nothing worse then an overcrowded ?Snowbird? infested restaurant. Unlike in the huge restaurant chains, Iris?s is somewhat prepared for the fluctuations of people. One can tell this because of the different lengths in waiting lines. At these large food chains the time triples to about an hour to an hour and forty-five minutes and at Iris?s it is just five to ten minutes longer than usual. It is the worst thing in the world to be sitting there with my stomach in a knot waiting in line to be seated after an extremely tough practice. The staff also has to put up with the stiff tipping and hard talking ?Snow Birds? that say, ?I want my seat right away.? I like the atmosphere best when the crowd keeps their gossip to themselves. Only because they speak loud enough that whole restaurant can hear there obnoxious, irritating, and deafening voices that say I want my ?Snow Bird Feed.? None of the regulars can stand these ?Snow Birds? either.

To me, Iris?s is un-comparable to any other franchise restaurants in town because of its simple hometown atmosphere that can take care of any customer. I can really appreciate the cleanliness and the great company of the regular customers also. The food there is also another key ingredient to my enjoying the restaurant so much. Nothing compares to the freshness of the food served here.

В 1988 году на ЗИЛе была выпущена первая партия рабочих станций «Беста»

В Советском Союзе делали суперкомпьютеры и мини-ЭВМ, преуспели в выпуске универсальных машин Единой Серии, в середине 80-х взялись за производство персональных компьютеров. Обо всем этом последнее время довольно часто вспоминают в разных компьютерных изданиях. И почти ничего не удалось разыскать мне в бумажных и сетевых компьютерных СМИ о первой советской Unix-станции. А такая разработка была, и вполне успешная. В 1988 году на созданном совместными усилиями Академии наук СССР и ЗИЛа новом производстве была выпущена первая партия рабочих станций «Беста-88».

Начало этой истории было положено в конце 70-х, когда группа сотрудников Московского университета в рамках хоздоговорных работ на ЗИЛе стала активно заниматься задачами машинной графики. Использовавшаяся в то время на заводе система прочностного расчета методом конечных элементов не предоставляла никаких средств визуализации не только результатов, но даже исходных данных. Приходилось вводить десятки тысяч цифр, что неизбежно порождало ошибки. Поэтому первой задачей, за которую взялись программисты-математики, была разработка графического визуализатора. Эффект внедрения новой системы был настолько впечатляющим, что разработчики с энтузиазмом продолжили работы. Владимир Борисович Бетелин, член-корреспондент РАН и директор академического НИИ системных исследований, возглавивший разработку «Бесты» со стороны Академии, вспоминает, что именно этот первый успех послужил толчком тому процессу, который в конечном итоге привел к созданию рабочих станций.

Решение проблем визуализации исходных данных и результатов моделирования постепенно перешло в работы по САПР. Поворотный для всей страны 1985 год оказался решающим и для совместной деятельности ЗИЛа, МГУ и Академии наук. В апреле 1985-го после визита на завод только что избранного Генеральным секретарем ЦК КПСС Горбачева было принято решение создать на базе ЗИЛа образцовое предприятие по системам автоматизации проектирования. До конца лета в напряженном режиме прорабатывались различные варианты реализации этой идеи. Для удовлетворения потребностей завода в САПР необходимы были 400 рабочих мест типа VAX-70 (графический дисплей с разрешением в 1 млн. пикселов и быстродействием 1 млн. операций в секунду). Когда, исходя из этих требований, просчитали вариант реализации на машинах ЕС и СМ, оказалось, что понадобятся средства, вдвое превышающие основные фонды ЗИЛа.

Середина 80-х — время появления в мире первых графических рабочих станций. Технико-экономическое обоснование проекта для ЗИЛа на базе подобных машин показало, что такое решение будет оптимальным. Даже при условии создания специального завода для сборки собственных рабочих станций такой проект обойдется значительно дешевле, чем использование существующей отечественной вычислительной техники. Бетелин рассказывает, что воплощение этой идеи в жизнь стало возможным благодаря активной поддержке академика Велихова, который в те годы был вице-президентом АН СССР, возглавлял в Академии Отделение информатики, вычислительной техники и автоматизации и курировал работы на ЗИЛе.

Со стороны ЗИЛа разработкой и производством рабочих станций руководил Александр Ильич Ставицкий. Весной 1988 года станция, получившая название «Беста», была продемонстрирована на общем собрании Академии наук. Первые десять машин собирались полуручным способом. Роботизация нового производства, автоматизация тестирования систем — все это было еще впереди.

Задумывая производство «Бесты», ее создатели руководствовались идеей использовать на заводе высококачественное импортное оборудование, лучшие западные производственные технологии и лучшие доступные на западном рынке лицензионные компоненты. А конечная цель — выпуск современных рабочих станций с хорошим показателем цена/производительность и высоким уровнем наработки на отказ. Была поставлена (и достигнута) почти фантастическая по тем временам планка — 10 тыс. часов бесперебойной работы.

По словам Велихова, это стало реальностью в следующем, 1989 году, когда на территории ЗИЛа под строгим личным контролем директора завода Сайкина для производства станций было построено специальное здание, внедрены роботы и полный цикл тестирования. По словам Велихова, получившийся в результате компактный завод вполне отвечал мировым стандартам и никак не уступал производству Apple, например. А Бетелин не без юмора вспоминает, что организация каждого элемента нового производства представляла собой мини-эпопею, вполне научную проблему, достойную академических умов: «Когда стабильно пошел процесс, это воспринималось нами как невероятная победа».

Долго обдумывался выбор операционной системы для «Бесты». Остановились на Unix, причем опять пошли по нетипичному для тех времен пути легального переноса лицензионной версии от AT&T. Правильность выбора подтвердилась через пару лет, когда первая графическая рабочая станция советского производства проходила апробацию в германском отделении корпорации Control Data. «Беста» с успехом функционировала в сети Ethernet, и CDC официально подтвердила, что советская разработка является стандартной Unix-станцией. «Беста» строилась на вполне современном для тех лет процессоре Motorola 68020. В качестве магистральной шины была взята VME, которая до сих пор остается индустриальным стандартом. Опора на стандартные технологии позволяла развертывать на «Бесте» разнообразные программные системы, накопленные в мире Unix.

Создание «Бесты» не сводилось только к разработке некоей экзотической аппаратуры или программного обеспечения. Это была комплексная проблема построения рабочей станции, на которой можно развернуть САПР и затем встроить систему в процесс проектирования на конкретном предприятии. Хотя, конечно, применялась «Беста» не только на ЗИЛе. Всего мини-завод выпустил несколько тысяч рабочих станций. По собственному опыту знаю, что это была действительно хорошая машина. Еще совсем недавно «Беста», например, работала в издательстве «Открытые системы».

Поиски западного партнера в 1990-1991 годах были связаны с надеждами создателей «Бесты» сделать свое детище более масштабным. Но экономические перемены начала 90-х, породившие серьезные проблемы для ЗИЛа, и открытие нашего рынка для западных производителей RISC-компьютеров поставили под сомнение перспективы дальнейшего производства «Бесты». С 386-ми персоналками «Беста» благодаря более мощной шине еще могла конкурировать, но соревнование с появившимися на российском рынке современных компьютеров НР и Sun в то время не представлялось реальным. Поэтому было принято решение переориентировать производство на более узкие ниши зарождающегося компьютерного рынка.

Разработка рабочей станции способствовала появлению сильной команды, в которую помимо программистов-математиков постепенно влились инженеры-практики из МИФИ. Овладев современными технологиями проектирования, они научились делать качественную серийную продукцию. Накопленный опыт пригодился в дальнейшем, когда созданный для «Бесты» завод перешел на выпуск специализированных систем на базе шины VME. Эта деятельность продолжается и по сей день.
